Call for Poster Presentations - Abstract Submission Instructions
Please submit abstract submissions to Sue Edstrom at Creative Health Care Management: sedstrom@chcm.com
The primary presenter of the abstract must download and complete the form below. No submission will be considered without this form. Creative Health Care Management (CHCM) will correspond with only one author.
Please save a copy of this form for your records. Abstracts must be no longer than 2 pages. Submissions longer than 2 pages will not be accepted for review. Deadline for submission is December 15, 2008. Applications which arrive after the deadline will not be accepted.
The abstract must be blinded. This means do not include the name of your organization or any individual's name in the body of the objectives or abstract. Failure to do so could result in your submission not being accepted for review.
