Professional Practice —
Relationship-based, Resource-driven Nursing Care

Professional Nursing Practice
The delivery of professional nursing services within the context of an organized care delivery model is a multifaceted process. The foundation for a professional practice model is laid when the professional nurse takes responsibility for establishing a therapeutic relationship with the patient and making decisions about the nursing care the patient will receive. Resources are used efficiently when the principles of professional practice and care delivery are clearly delineated and articulated.

Implementation
CHCM provides the consultation and education services to assist your organization in developing a principle-based, resource-driven, and outcome-oriented professional practice care delivery model, customized to meet the needs of your organization. A variety of services are available, from workshops on elements of professional practice to a comprehensive consultation to design, implement and continuously improve your care delivery.

"Until patients in hospitals routinely receive coordinated care, delivered with continuity over time and place, managed by a RN known to the patients and families as the responsible nurse, this profession will continue to fall short of its promise and obligation."

- Marie Manthey
